Changes to some service type descriptions

In August 2008 the HACC service type descriptions below were updated to bring them in line with NSW Guidelines for HACC funded services released in 2007. The service type description for Social Support - Dementia Monitoring was also renamed Social Support - Monitoring. It now covers dementia monitoring and health monitoring, including electronic monitoring. The Service Type Guidelines for Social Support were also updated.
