This site contains information and resources relevant to the collection, entering and transmission of MDS data for all organisations in NSW that receive funding under Home and Community Care (HACC). HACC Programs are NSW’s principle support services for frail aged people, younger people with disabilities, and carers.
This collection of data occurs on a quarterly basis in line with your funding cycle and provides service user profiles and details of the type and amount of assistance provided. The Australian Government and State and Territory Governments use this information to help plan for the HACC Program.
